Your Pet's First Visit, on One Page

Everything worth having in hand for a first appointment at Blue Cross. Print it, stick it on the fridge, and come see us.

Bring What You Have. Skip What You Don't.

None of this is required. If all you bring is your pet, come anyway and the record starts fresh.

Records from your previous vet, if you have them. Bring them along or have them sent over, and they get folded into your pet's one record here so nothing gets lost.
A list of any medications your pet takes, with doses if you know them.
The food your pet eats. The brand and type is enough; a photo of the bag works.
Anything that has been worrying you. A lump, a limp, a change in appetite or behavior. Write it down so it does not slip your mind in the room.
Your questions. There is no rush in the exam room, so bring the whole list.
For the ride: dogs on a leash, cats in a carrier, and everyone gets there calmer.

Cost is discussed before anything is done. If something can wait, we say so.
